Résumé

Objectif : Le Syndrome de Déficience Posturale (SDP) est un dérèglement du Système Postural d’Aplomb. Il résulte d’une dysproprioception qui engendre une dysperception corporelle et cognitive. Les signes psychomoteurs associés renvoient à des difficultés scolaires incluant l’apprentissage de la lecture. La dyslexie associée au SDP est traitée par reprogrammation proprioceptive. Quel est l’apport de l’ostéopathie dans le traitement proprioceptif de l’enfant dyslexique tant sur le plan postural que cognitif ?

Méthodes : 24 patients dyslexiques atteints de SDP ont été randomisés en deux groupes : 12 traités en ostéopathie ; 12 non traités. Durant six mois, leur évolution a été objectivée par deux tests posturaux réalisés tous les deux mois : Épreuve Posturodynamique et Manoeuvre de Rotation Cervicale. Parmi eux, 16 patients ont été évalués en orthophonie dont huit de chaque groupe. Leur évolution a été objectivée par trois tests psychométriques réalisés avant et après le traitement à six mois : Test d’Alouette, Dictée de mots, Épreuves Métaphonologiques.

Résultats : Les différences inter groupes sont très hautement significatives à l’Épreuve Posturodynamique et la rotation cervicale gauche. Les différences intra groupe traité sont hautement significatives au Test d’Alouette, la Dictée de mots réguliers et pseudo-mots et l’Épreuve Métaphonologique de fusion.

Conclusion :L’ostéopathie rétablit les capacités posturocinétiques qui normalisent le SDP et améliore les tâches cognitives de l’apprenti lecteur. Elle participerait au développement des fonctions cognitives. Il conviendrait d’inclure une épreuve de lecture de mots, et d’intégrer l’orthodontie neurosensorielle dans la reprogrammation proprioceptive. Cette étude met en lumière la nécessité d’agir en étroite collaboration pluridisciplinaire autour de la dyslexie.

Abstract

Objectives: The Postural Deficiency Syndrome (PSD) results from a disturbance in the balanced postural system. A dysproprioception, leads to physical and cognitive dysfunctional perception. Combined motor and cognitive signs get back to learning difficulties including in literacy skills. Dyslexia associated with PSD is treated by proprioceptive reprogrammation. Can osteopathy be of interest in the proprioceptive development of the Dyslexic child on postural as well as cognitive aspects?

Méthods: 24 dyslexic patients presenting with a PDS were randomized in two groups: 12 treated with osteopathy, 12 not treated. Their postural evolution was studied during 6 months using 2 postural tests applied once every two months: Posturodynamic test and Cervical rotation maneuver. Among these patients, 16 were tested for speech-language dysfunction, 8 in each group. Their cognitive evolution was studied with three psychometric tests applied before treatment and at six month, after treatment: Alouette, words’ dictation and meta-phonologic tests.

Résults: We found very highly significant intergroup differences in the posturodynamic and left Cervical rotation tests. The intragroup differences are highly significant in the treated group to the Alouette test, regular words and pseudo-words dictation and significant in phonemes fusion test.

Conclusion: Osteopathy optimizes the postural and cinetic abilities’ regulation, normalizing the PDS and seemingly improving the cognitive tasks of the readers to be. Osteopathy could well be participating to the cognitive function development. Proprioceptive reeducation should include a word reading test and sensorial orthodontics. This study brings on the need to act conjointly in a multidisciplinary approach to Dyslexia.
